Health Requirements

 

Medication

Administration of Medication During School

 

Any student requiring medication (including over-the-counter drugs, cough drops, ear and eye drops) during the school day must be under the direct supervision of the school nurse. The procedures are in accordance with statutory regulations outlined by the Nurse Practice Act and Provision of the State Education Law.

 

1. Medication is to be delivered to the health office by the parent/guardian and it will be stored in a locked area.

 

2. Medicine is to be in the original pharmacy bottle with the pharmacy label intact, the first and last name of the student, name of the drug, the dosage, and the frequency of administration.

 

3. A note from the physician must accompany the medication, stating name of the student, name of the drug, the dosage, time of administration and the reason for giving the medication.

 

4. A note from the parent/guardian requesting that the medication be given by the school nurse as prescribed by the physician must also be submitted.

 

 

Physical Examinations - PreKindergarten

 

The physical examination for Pre-K students must be submitted before the student has been in the Pre-K program for 30 calendar days. A private physical examination may be accepted if it has been done not more than twelve months prior to the commencement of the school year in which the examination is required.

 

The following must be provided for any child entering a Pre-K program:

 

• a report of a medical examination, signed by a licensed health provider, stating that the child is free of contagious or communicable disease

• a record of the child’s immunizations demonstrating compliance to Section 2164 of the Public Health Law, or has a medical or religious exemption from such immunizations

• a record that the child has had a blood lead test prior to or within 3 months of initial enrollment.

 

A complete list of Pre-K requirements can be obtained from the Nurse’s Office.

 

Physical Examinations - Grades K - 12

 

In accordance with the Cleveland Hill School District policy and the New York State regulations, all students in kindergarten, second, fourth, seventh and tenth, plus all new students entering our school district are required to have a physical examination and requires each student’s health certificate or health appraisal to include an assessment of Body Mass Index (BMI) and weight status (based on BMI-for-age percentile). Special education students may need a physical examination as part of their triennial evaluation. Also, all those students who participate in school sports must have a yearly physical examination. This examination may be completed by the child’s own physician or by the school doctor. Students who do not provide proof of a physical examination by a personal physician will be examined by the school doctor at the beginning of the school year.

 

Education Law Sections 903 & 904

 

NYS Immunization Policy

 

New York State Public Health law, Section 2164, mandates that schools not permit a child to be admitted unless the parent provides the school with a certificate of immunization or proof from a physician that the child is in the process of receiving the required immunizations. The required immunizations are:

 

• 3 doses of Diphtheria Toxoid (usually administered as DPT, DT or TD)

• 3 doses of Oral poliovirus vaccine (OPV and/or IPV)

• 2 doses of Measles vaccine and 1 dose each of Mumps and Rubella. The first dose of Measles, Mumps, and Rubella (MMR) must be administered no more than 4 days before the first birthday   

• 3 doses of Hepatitis B vaccine

• 1 dose of Diphtheria and Tetanus toxoid containing vaccine and acellular

• Pertussis vaccine (Tdap) for all children born on or after 1/1/94 who are entering the 6th grade

• 1 dose of Varicella Administered no more than 4 days before the first birthday. Applies to all children born on or after 1/1/98 or all children born on or after 1/1/94 who are entering the 6th grade.

 

All of the above immunizations must be documented by your physician or local health department where the child received the immunization or must be from an official copy of the immunization record from the child’s previous school. All immunizations must specify the exact date each immunization was administered.

 

FAILURE TO COMPLY WITH THESE REGULATIONS WILL RESULT IN EXCLUSION OF THE CHILD FROM SCHOOL.
