Photo, Video, and Internet 'Opt Out'

The Cleveland Hill School District's policy for photographs and Internet usage during school hours states that all students are considered permitted to have pictures used in printed materials and on the Internet unless the District receives the form attached below.

There are also times when the District posts videos highlighting our students involved in scholastic and athletic activities. This form must be completed and returned if you wish that videos and/or pictures with your child in them not be posted on the school's web site.

This form must be completed and returned to your child's school principal.

Please allow 7 to 10 days for processing the request after receipt.

Photo, Video, and Internet 'opt out' (pdf file)
This form permits you to ”opt out” of the having your child's photos used on printed school materials, in video, or on the web. You must also complete and return this form if you wish your child NOT to use Internet resources when at school.
